Ingredient 5 of 5

Huperzine A in SonoVive: A Dementia-Drug Look-Alike, Explained

Quick answer

Huperzine A is an alkaloid from Chinese club moss and the last of SonoVive's five named ingredients. It works like dementia drugs such as donepezil. Alzheimer's trials used 200-800 mcg a day and were of poor quality (Yang 2013 huperzine meta-analysis). It was never tested for hearing. Sponsored articles report 10 mcg per capsule, unconfirmed.

The basics

What is huperzine A?

Huperzine A is a plant alkaloid taken from Chinese club moss, Huperzia serrata. It acts on the brain much like a prescription dementia drug.

SonoVive names it last of five ingredients, and the sales page mentions it by name: "...Huperzine-A and more!". The vendor gives no amount. It also never explains what a memory compound is meant to do for hearing. The bottle's Supports Brain Function claim is the closest link.

The profile shows how little the seller publishes next to what research used.

In the body

How does huperzine A work?

It slows an enzyme that breaks down acetylcholine, a chemical messenger nerve cells use for memory and attention. Drugs that do this are called cholinesterase inhibitors (ADDF huperzine A).

Donepezil and rivastigmine, two common Alzheimer's prescriptions, work the same way. That is why huperzine A is studied in Alzheimer's disease and not in ear problems. It is also why it behaves less like a gentle herb and more like a medicine.

Why this matters for your parent's pill box

If Mom already takes donepezil, SonoVive would add a second product with the same action. The ADDF says to avoid huperzine A with donepezil, rivastigmine and similar drugs (ADDF huperzine A).

Evidence, ranked

Is there good evidence for huperzine A?

  1. Hearing aids
The proven help for age-related loss

Hearing aids, including over-the-counter aids for adults with mild to moderate loss, are the standard treatment. (FDA OTC hearing aids)

  2. Bacopa
    Memory trials, 12 weeks

Some gains in memory recall at 300-450 mg a day. Never tested for hearing. (Pase 2012 bacopa review)

  3. Huperzine A
    20 Alzheimer's trials

Some score gains, but poor study quality. Never tested for hearing. (Yang 2013 huperzine meta-analysis)

  4. Ginkgo
    Cochrane 2022: 12 tinnitus studies

Uncertain benefit; it may make little or no difference to how bad ringing feels. (Cochrane: Ginkgo for tinnitus 2022)

  5. Vinpocetine
    Cochrane: 3 dementia studies

Inconclusive, and FDA warns against it in pregnancy. (Cochrane vinpocetine)

  6. SonoVive
    No study of the product

No trial of SonoVive for hearing, ringing or memory has been published. (FTC health products guidance)

Bar length shows how directly each line of evidence answers the question: will this help my hearing? It is our editorial summary of the linked sources, not a score from any study.

The evidence is weak. A 2013 analysis of 20 Alzheimer's trials found better memory test and daily-living scores, but warned the results should be read with caution because the studies were poorly done (Yang 2013 huperzine meta-analysis).

The studies

What did huperzine A trials actually find?

Small gains in people who already had Alzheimer's, in trials that reviewers did not trust much. An earlier Cochrane review of 6 trials with 454 patients found only one adequate study and called the evidence "insufficient" to recommend use (Cochrane huperzine A).

None of these trials enrolled healthy older adults who simply want a sharper memory. That is a different group from most SonoVive buyers.

Dose check

How much huperzine A is in SonoVive?

The vendor does not say. Two sponsored articles report 10 mcg per capsule, and we could not confirm it.

If that number is right, one capsule holds 5% of the lowest Alzheimer's trial dose of 200 mcg a day (Yang 2013 huperzine meta-analysis). It is a tenth of the 100 mcg low end used in vascular dementia (ADDF huperzine A). Watch the units: mcg means micrograms, a thousandth of a milligram. The two units are easy to mix up: 0.01 mg and 10 mcg are the same amount.

A tiny amount is no reason to take more. Follow the directions printed on your bottle, and see the SonoVive dosage page for why we print no dose.

Safety

What are the side effects of huperzine A?

Trials report nausea, loss of appetite and dizziness (Yang 2013 huperzine meta-analysis). The ADDF adds headache and blurred vision, and says long-term safety has not been studied (ADDF huperzine A).

Common

Stomach and appetite

Nausea and eating less were the most frequent complaints in trials (Yang 2013 huperzine meta-analysis).

Head and eyes

Dizziness and blurred vision

Worth noting for anyone who drives, climbs stairs or has had a fall (ADDF huperzine A).

Unknown

Years of use

Trials lasted up to 36 weeks. Nobody knows what happens over many years (Yang 2013 huperzine meta-analysis) (ADDF huperzine A).

Taking donepezil or rivastigmine?

Do not add SonoVive without your prescriber's okay. Two products with the same action may add up to more side effects, such as nausea and dizziness.

SonoVive also names bacopa, which the ADDF lists as a possible interaction with this drug class (ADDF bacopa 2024). Read about that overlap on the bacopa page.

Ears, not memory

Can huperzine A help with ringing ears or hearing loss?

No trial has tested it for either. Every study we found enrolled people with dementia and measured thinking and daily living.

Picture a 64-year-old who started hearing a hiss after a bad cold and now buys hearing products online. Huperzine A gives him nothing proven for the hiss. His first step is a doctor's visit, and if the hiss is in one ear only, sooner rather than later (FDA OTC hearing aids).

Which other ingredients does SonoVive name?

Huperzine A is the last of five. Start again with ginkgo, the only one tested for tinnitus, or read St. John's wort, the one most likely to clash with your medicines.

FAQ

Huperzine A in SonoVive: common questions

How much huperzine A is in SonoVive?

The vendor does not publish it. Two sponsored articles report 10 mcg per capsule, which we could not confirm. Alzheimer's trials used 200-800 mcg a day, so the reported amount is about 5% of the lowest dose studied. Follow the directions printed on your bottle.

Can I take huperzine A with donepezil?

No, not without your prescriber's approval. Huperzine A works the same way as donepezil and rivastigmine by slowing the breakdown of acetylcholine. The ADDF advises against combining them because the two act on the same enzyme and their effects may add up.

Does huperzine A help tinnitus?

There is no evidence it does. Every huperzine A trial we found studied people with Alzheimer's or vascular dementia, not ringing in the ears. NIDCD points to sound therapy, hearing aids and counseling for tinnitus and says no supplement has been proven effective for it.

Is huperzine A safe to take every day?

Short-term use looked safe in trials lasting up to 36 weeks, but long-term safety has not been studied. Reported side effects include nausea, loss of appetite, dizziness, headache and blurred vision. Check with your doctor first, especially if you take any dementia medicine.

Is huperzine A a drug or a supplement?

In the U.S. it is sold as a supplement, but it acts like the prescription Alzheimer's drugs called cholinesterase inhibitors. That is why it carries drug-like interaction warnings. It comes from Chinese club moss, Huperzia serrata.

Does huperzine A improve memory in healthy adults?

The research does not answer that. The trials we found enrolled people who already had Alzheimer's or vascular dementia, and even there the 2013 review called study quality poor. A Cochrane review found the evidence insufficient to recommend it.

What is the difference between 10 mcg and 0.01 mg of huperzine A?

There is none: 10 micrograms equals 0.01 milligrams. Product pages may use either unit, so convert before you compare. The 10 mcg figure for SonoVive comes from sponsored articles, not from a published Supplement Facts panel.
