St. John's Wort Drug Interactions: A Plain Guide for SonoVive Buyers
St. John's wort, one of SonoVive's five named ingredients, can make many medicines work less well. NIH lists warfarin, birth control pills, cyclosporine, some statins and antidepressants such as amitriptyline and bupropion. With drugs that affect serotonin it may cause serious side effects (NCCIH St. John's wort). If you take any daily medicine, ask your pharmacist before the first capsule.

Why it matters
Why does St. John's wort matter for SonoVive?
Because it is the ingredient most likely to clash with what is already in your pill box. St. John's wort is a yellow-flowered herb best known as a mood remedy. It has never been studied for hearing.
The vendor does not publish how much is in each capsule. Two sponsored articles report 250 mg, which would make it the largest ingredient, but we could not confirm that figure. Either way, the herb is in the formula, and the interaction risk comes with it.
Hearing supplements are sold mostly to people over 60, who often take several daily medicines. They have the most to lose from a weakened prescription.
The list
Which medicines does St. John's wort interact with?
NIH's complementary health center names these. Your pharmacist can check the rest of your list.
| Medicine type | Examples named | What could happen |
|---|---|---|
| Birth control pills | Oral contraceptives | Less protection against pregnancy (NCCIH St. John's wort) |
| Blood thinner | Warfarin | Weaker effect, which raises clot concerns (NCCIH St. John's wort) |
| Transplant medicine | Cyclosporine | Lower drug effect, a serious risk after a transplant (NCCIH St. John's wort) |
| Cholesterol medicine | Some statins | Less cholesterol control (NCCIH St. John's wort) |
| Antidepressants | Amitriptyline, bupropion | Weaker effect (NCCIH St. John's wort) |
| Drugs that affect serotonin | Many antidepressants and some other drugs | Possible serious serotonin-related side effects (NCCIH St. John's wort) |
| HIV medicine | Ask your HIV team | On our ask-first list; do not combine without advice |
In plain words
How does St. John's wort weaken other medicines?
In short, it can lower how much of a medicine stays active in your body. The drug is still taken on time, but less of it does its job.
That makes the problem easy to miss. Nothing feels wrong on day one. A blood thinner may lose its grip, or a birth control pill may stop protecting, and the trouble shows up later (NCCIH St. John's wort).
Antidepressants
Can you take SonoVive if you are on an antidepressant?
Not without your prescriber's okay. St. John's wort is itself studied as a mood herb. A review of 35 trials with 6,993 patients found it beat placebo for mild to moderate depression, though the authors urged caution in reading the results (Apaydin 2016 SJW review).
That is the point. Adding a mood herb on top of a mood drug can stack effects. NIH warns that with drugs that affect serotonin it may cause serious side effects (NCCIH St. John's wort).
Picture a 58-year-old on an antidepressant who has ringing in both ears. She sees a SonoVive bottle, reads Promotes Hearing Health, and thinks of it as an ear product. Nothing on the front mentions mood or St. John's wort.
Her safest path: take the bottle to her prescriber or pharmacist and ask the direct question before she opens it.

What should you ask your pharmacist about St. John's wort?
Bring the bottle and every medicine you take, including over-the-counter ones. Then ask:
Does St. John's wort affect any of my medicines?
Name each one. Include birth control, blood thinners, statins and anything for mood, sleep or pain.
Is there a safe way to try this at all?
For some people the answer is simply no. That is a useful answer.
What should I watch for?
Ask which signs mean stop and call. Get the answer in writing if you can.
Tell them the vendor publishes no Supplement Facts panel, so the dose is unknown.
On its own
What side effects can St. John's wort cause by itself?
Apart from interactions, NIH lists diarrhea, dizziness, trouble sleeping, restlessness and skin tingling. At large doses, some people get severe skin reactions after sun exposure (NCCIH St. John's wort).
If any of these appear, stop and call your doctor. You can also report a problem to FDA MedWatch at 800-FDA-1088 (ODS: What you need to know) (FDA MedWatch). More detail on the SonoVive side effects page and the St. John's wort ingredient page.
Your next step
What are safer first steps for ringing in the ears?
A hearing test and a doctor visit carry no drug interactions at all. Hearing aids, sound therapy and counseling are the helps NIH lists for tinnitus (NIDCD tinnitus). See what actually helps tinnitus and SonoVive vs hearing aids.
If your pharmacist clears it, follow the directions printed on your bottle.

FAQ
St. John's wort and medicine questions
Does SonoVive contain St. John's wort?
Yes. St. John's wort is one of the five ingredients the brand names. The vendor does not publish the amount; sponsored articles report 250 mg per capsule, which would make it the largest ingredient, but that figure is unverified.
Can St. John's wort make birth control fail?
It can make birth control pills less effective, according to NIH. If you rely on the pill, talk to your doctor before taking any product with St. John's wort, including SonoVive, and ask about backup protection.
Can I take SonoVive with warfarin?
Ask your doctor first. St. John's wort can weaken warfarin, and ginkgo and vinpocetine, also named in SonoVive, may raise bleeding risk. That mix of effects is hard to predict without your care team.
What happens if you mix St. John's wort with an SSRI?
It may cause serious serotonin-related side effects, NIH warns. Many common antidepressants affect serotonin. Do not combine them on your own; your prescriber needs to decide.
Does St. John's wort help hearing or tinnitus?
No study shows that it does. St. John's wort has been tested for depression, in 35 trials with 6,993 patients, but never for hearing or ringing in the ears.
How long before surgery should I stop SonoVive?
Ask your surgeon; the vendor gives no guidance. NIH notes that some supplements change bleeding risk or anesthesia response, and SonoVive names ginkgo and St. John's wort. Tell the surgical team about every supplement you take.
